Office Hours

Our office hours are 7:30 a.m. to 5 p.m., Monday through Friday. Be assured that we provide emergency coverage 24 hours per day, 7 days a week.

Initial Visit

On your initial visit, please bring the following items:

• Identification
• Insurance card(s)
• Written referral form, if required
• List of medications you are taking
• Pertinent test results
• Completed pre-registration form: English | Spanish (PDF)

Also, be prepared to share your medical history with our physicians. This will include any medical conditions you have or may have had in the past, surgical history, any medications (prescription or over-the-counter) you are currently taking (include name, dosage and frequency), and any allergies you have to medications or foods.

Directions

From South or Downtown Atlanta:

Take I-85 North which will combine with I-75 North (75/85 North).
Take Exit #249B - Peachtree/Pine Street (Crawford Long Hospital) and stay in the left lane.
Turn Left onto Peachtree Street at the first traffic light.
Turn Right onto North Avenue

Midtown Urology is on the left corner of North Avenue and Juniper Street at the first traffic light. The parking lot is on the side of the building. Park in any space that says 'Visitor Parking for the 128 Building.' Then get a parking pass at the front desk in our office to place in the window of your car.

From North of Downtown Atlanta:

Take I-75 South (from Cobb County) or I-85 South (from Gwinnett County).
Take Exit #249D - North Avenue and turn left.

Midtown Urology is on the left corner of North Avenue and Juniper Street at the fourth traffic light. The parking lot is on the side of the building. Park in any space that says 'Visitors parking for the 128 Building.' Then get a parking pass for your car at the front desk in our office to place in the front window of your car.

From East of Downtown Atlanta:

Take I-20 West to I-75/I-85 North towards Chattanooga-Greenville.
Take Exit #249B - Peachtree/Pine Street (Crawford Long Hospital) and stay in the left lane.
Turn left onto Peachtree Street at the first traffic light.
Turn right onto North Avenue.
